Idaho's flat tax changed the math for every practice owner

Idaho taxes individual and corporate income at a flat 5.3%, down from 5.695% under House Bill 40, signed in March 2025. A flat rate sounds like it takes the planning out of the equation. It does the opposite: it moves the decisions to owner compensation, retirement contributions, equipment timing, and quarterly estimates. If your plan predates the rate change, it was built for a law that no longer applies.

If your practice is an S-corp or a partnership, Idaho's Affected Business Entity election lets the entity pay Idaho income tax at the corporate rate and gives each owner a credit against their personal Idaho tax, a workaround for the federal SALT deduction cap. It has to be elected every year on the return, and it carries a trade-off worth understanding before you make it: an electing entity cannot claim the Idaho capital gains deduction on the sale of Idaho property and has to pass that deduction through to its owners instead, according to the Idaho State Tax Commission. That matters if a building sale or a practice transition is anywhere on your horizon.

Idaho is still an owner's market, too. About 12% of Idaho dentists are affiliated with a DSO, against roughly 16% nationally, according to the ADA Health Policy Institute's 2024 workforce data. The owners who run on real numbers are the ones who keep it that way.
