Claiming Your Share of the $5.54 Billion Credit Card Settlement

If your dental practice accepted Visa or Mastercard payments between 2004 and 2019, you could be eligible to claim part of the massive $5.54 billion Payment Card Settlement.

  • Your business must have accepted Visa or Mastercard payments during the qualifying period (2004–2019).
     
    • If you sold or closed your practice , you are still eligible as long as your practice accepted Mastercard or Visa any time between 2004-2019.

    • Most dental practices meet this requirement.

You will need:

  • Your Taxpayer Identification Number (TIN) or Employer Identification Number (EIN).

  • Your Claimant ID and Control Number, which are included in the mailed claim form (if you received one).

    • If you didn’t receive a mailed claim form (or can't find it), don’t worry, you can still file online or request assistance​.

Filing Your Claim

1. Access the Claim Form

Visit PaymentCardSettlement.com to access the online claim form. If you didn’t receive a mailed form, you can download one or request assistance by calling 1-800-625-6440. The website provides user-friendly instructions to guide you through the process.

2. Gather Proof of Authorization

Ensure you have the following document ready before starting your claim:

  • Proof of Authorization (can use any of the following):
     
    • EIN Assignment Letter from the IRS.

    • A completed W-9 Form to validate your business’s tax details.

    • Recent Utility Bills showing your business’s name and address to confirm operational status.

3. Register or Log In to the Claims Portal

If you have a Claimant ID and Control Number from a mailed claim form, use it to log in. If not, register your business by providing your TIN or EIN. This step ensures your practice is correctly identified in the system.

  • Representative Name = Your Name.

  • Representative Title = Your Title.

  • Representative Business = Your Practice.

4. Verify Estimated Interchange Fees

The portal will display estimated interchange fees based on your transaction history from 2004 to 2019. Here’s how to proceed:

  • If the estimate looks accurate, Confirm and proceed with the claim.

  • If the estimate seems incorrect, Request additional transaction details. You may need to upload supplemental documentation, such as sales records or merchant statements, to justify adjustments.


5. Complete and Submit Your Claim
Follow these steps to finalize your claim:

  • Review all pre-filled information and confirm accuracy.

  • Upload your required documentation as outlined in Step 2.

  • Submit the claim online through the portal. Alternatively, you can print the claim form, complete it manually, and mail it with your supporting documents to:

6. Keep Track of Deadlines

The final deadline to submit your claim is February 4, 2025. Late submissions will not be considered. Filing early ensures faster processing and avoids any last-minute issues.

7. Confirm Submission

Once you submit your claim, save a copy of the confirmation for your records. For mailed claims, consider using a service that provides delivery confirmation.
